Advanced Seating & Mobility (ASM) values your privacy and will protect your health and medical information.
ASM has set rules and limits on who can look at and receive your health information.

ASM protects information that has been given to us by your doctors, nurses and other health care providers which is used in providing you with service related to our business.

ASM protects and does not share conversations its staff personnel may have with outside sources.

ASM protects your individual information by using a secured computer system.

ASM will use your health information in order to pursue equipment approval from a funding source.

ASM will not share or sell your health information as a marketing tool.

As a client – you have the right to:

• Ask to see and get a copy of your health records
• Have corrections added to your health information
• Decide if you want to give your permission before your health information can be used – this permission is given to ASM by signing the HIPAA paperwork
• If you believe your rights are being denied or your health information isn't being protected you can file a complaint with ASM (see complaint section) or by calling the U.S. Government at 1-866-627-7748
